Agency-managed, plan-managed and self-managed. Funding arrangements are confirmed before supports start so invoicing matches the plan.

What to include in a referral

  1. Participant first name and preferred contact.
  2. Plan goals relevant to the support requested.
  3. Funding arrangement and plan dates.
  4. Suburb and preferred days or times.
  5. Any clinical, behavioural or cultural considerations.
  6. Risk information relevant to worker safety.

How to refer
